This website page contains the CMS Cost Report Procedures and Forms. This documentation was obtained directly from the CMS website. The documentation is limited to the current procedures and forms. The old CMS Procedures and forms can be found on the CMS Website in the Cost Report section. In general, the CMS documentation is updated when CMS receives new rules or regulations for the Medicare/Medicaid Program. The changes are highlighted in red itialics with a date of change.

This is the current Hospital Cost Report documentation in PDF Format obained directly from the CMS Website. This new CMS documentaiton was released in 2010. The old CMS Form 2552-1996 is available on the CMS website. The procedures are in .Docx format in 6 files. The forms are in an .xlsm file on the CMS website.

This is the current HHA Cost Report documentation in PDF Format obained directly from the CMS Website. This new CMS documentaiton was released in 1996. This is the only CMS Cost Report Documenation that has not been upgraded.
